First, you will need a General Surgeon's Letter:



I, (physician’s full name), (physician’s medical license or certificate number), (issuing U.S. State/Foreign Country of medical license/certificate), (DEA Registration number or comparable foreign designation), am the physician of (name of patient), with whom I have a doctor/patient relationship and whom I have treated (or with whom I have a doctor/patient relationship and whose medical history I have reviewed and 
evaluated). (Name of patient) has had appropriate clinical treatment for gender transition to the new gender (specify new gender male or female). I declare under penalty of perjury under the laws of the United States that the forgoing is true and correct.

This is usually written by the surgeon who has done your SRS (Sexual Reassignment Surgery), however, some have succeeded in changing their gender marker without surgery with the assistance of a co-operative medical doctor.

Next,


You will need to take the Surgeon's letter to your local DMV. The best place to go in this area would be the Elizabethton DMV. The office workers follow protocol and do not let personal feelings get in their way. They will take your letter and will have you fill out the usual documentation. Then, they will have you wait while they get everything on their end situated. Your number will be called, you will pay the usual fee, and then you will have your picture taken. Congratulations on your new appropriately gendered I.D.!
